Glad to hear the trip was a success. I like the Q&A format of this trip report.
For anyone reading this report consider hiring a private driver for the day when you climb Malinche. They will take you to Malinche, wait for you with your gear, then on to Tlachichuca.
There are also quality hotels in both Mexico City and Puebla in the 30-70USD range that are well worth considering.
Cheers!

Awesome!
I'm attempting Orizaba the first week of January with CAG. My first mountain higher than Whitney. I found your post to be very informative. Thanks!
That time (in 2008) it was $150 (USD) per person, which included: lodging, breakfast, dinner, water in big bottles (to take them up), gas containers, transportation for 3 peoples from Tlachichuca to Piedra Grande and back. It must be more expensive by now.
February 8, 2016 on the standard Jamapa Glacier route:
I was making a solo ascent of the peak, convinced that crevasse danger was minimal from all I had read online. I broke through the surface of the glacier at 18100', fell 8 meters to the crevasse floor, and spent the next 24 hours trying to get out of a 3 meter wide crevasse. I was eventually rescued. I feel compelled to advise climbers that this mountain is changing. Large crevasses do exist. One nearly took my life. I will return to Orizaba, but only as part of a roped team. In the spirit of common sense, I invite others to rope up.
I was there the following week. You were still the talk of the town. One of the guides in the rescue party told me they expected to retrieve a corpse from the crevasse but were positively impressed when your only complaint was that your mouth was a little bit dry ("Sólo se quejó de que tenía la boca un poco seca").
I'm climbing Orizaba this winter and was wondering if my boots would be warm enough. I have Salewa Rapaces. I bought them a little large and usually wear two pairs of socks.

La Malinche trailheadThere are actually several mini buses to and from the La Malinche trailhead every day. The cost is $1 US. The last one leaves the trailhead at 5 pm.
