Poor Penmanship

Going through some files I came across this form which I'd filled out in the hospital shortly after regaining consciousness. On the right is an example of how the same form would look today. I've always had very neat handwriting so it was kind of a shock to see how poorly I was writing after the accident. Photo submitted for the A Major Failure on the Beatout trip report.
